Skilled Health Care for Patients at Home

The focus of home care is to take care of the individual's health needs within the comforts of their own home, in a family setting. Activities are directed toward helping patients to attain and maintain their highest level of independent living and to help the family provide any necessary support. Efforts to prevent further disability, educate regarding the patient's health and offer appropriate referrals to allied health professionals are also emphasized.

How is eligibility determined?

Home Care is available to all, without regard to age, race, religion, sex, color, national origin, marital status, handicap status, organizational affiliation, physical or mental impairment, or financial status. If a need is determined, the patient's physician is responsible for recommending home care and for establishing a treatment plan, which is incorporated into the patient's record and maintained by Home Care staff. The physician must certify and acknowledge the following:

  • Home Care services are necessary.
  • A plan for treatment has been established.
  • The patient is in need of the various servics as outlined in the treatment plan.
  • The patient will not falsify or conceal essential information.

The physician will review the treatment plan periodically and re-certify that there is a continuing need for service.

How is a treatment plan developed?

After the patient's referral has been accepted, the Home Care staff will assist the physician in planning for the patient's home care. An evaluation is made to determine the patient's needs for nursing services, physical therapy, speech therapy, occupational therapy, medical social services and home health aides, as well as if special supplies or equipment are required. Family members are given an orientation and trained in their responsibilities to provide care.

What services are available through Home Care?

Under the direction of the patient's physician and as part of a patient's custom treatment plan, Home Care can provide the following:

  • Nursing - A professional nurse assesses the patient's condition to assure that health care needs are met and perform the skilled procedures ordered by the patient's physician.
  • Physical, Occupational and Speech Therapy - Therapists provide prescribed exercises to develop body strength and coordination as well as mastery of everyday skills.
  • Medical Social Work - Social workers assess social and emotional factors and assist with community resource planning as well as long-range planning and decision making.
  • Home Health Aide - Aides provide short-term services to assist patients with personal care needs, such as bathing; hair, nail and skin care; and dressing. Aides take and record vital signs and report any significant findings to the nurse assigned to the patient's care.
  • Telehealth - Telehealth monitoring system promotes patient independence in management of disease processes at home and can help avoid re-hospitalization. The system can also monitor weight, vital signs, oxygen saturation, etc., on a daily basis.

What counties can receive Home Care?

The Home Care service area encompasses the communities of 18 counties in the South-Central Illinois region. Home Care services are a collaboration between Sister hospitals - St. Anthony's Effingham, St. Joseph's Breese and St. Joseph's Highland - to provide home health services to this region and all its communities.
